Job Description:

The purpose of this classification is to perform duties related to providing minimal but collaborative healthcare, education, and social engagement services for residents that are enrolled in the adult daycare program.

Job duties may include but are not limited to:

  • Administering evaluation tools
  • Making recommendations
  • Identifying health related needs of candidates seeking services
  • Counseling and training caregivers, participants, volunteers and interns on specialized health care prevention and self-management promotion
  • Promoting preventative health education
  • Supervising and assisting participants with medication administration and health treatment needs
  • Observing and evaluating participant level of functioning and providing recommendations for level of care needed
  • Monitoring and enforcing disease control services within the facility
  • Implementing and managing standard precautions and infection control guidelines
  • Training and demonstrating standards of precaution to staff, interns, and volunteers
  • Performing inspections, screenings, teachings, and counseling in the area
  • Reporting all new suspected or confirmed cases of infection to the Department of Health and Wellness, Director of Epidemiology.
  • Participating in investigations of disease control efforts
  • Reviewing participant medical records and information to reduce the contraction of airborne diseases in the ADP area
  • Generating documented services and maintaining records for the program participants
  • Observing, monitoring, and participating in the evaluation and maintenance of stabilized health and functioning of seniors in the program
  • Promoting, teaching, and improving individuals desires and abilities to perform self-care or maintain care through the assistance of available staff
  • Evaluating and documenting current status of participant care
  • Generating, promoting, and executing participant and facility safety standards
  • Coordinating collaborations with sister programs to provide additional services if necessary
  • Facilitating coordination of individual care plans with service providers
  • Assisting the program coordinator in providing guidance and direction of adult daycare staff, contract staff, and volunteers / interns
  • Clarifying goals, objectives, policies, and procedures to all assigned personnel assisting in the program area
  • Advising and training new health care staff and aides to individualized participant programming and service delivery system
  • Demonstrating, evaluating, training, and coaching staff on performance of healthcare services
  • Reviewing, clarifying, and documenting health related documentation on all participants monthly
  • Reviewing, evaluating, improving, and facilitating the overall quality of service delivery to participants and caregivers
  • Preparing environment for daily activities (i.E. Changing the calendar, thought of the month, open doors, and etc.)
  • Coordinating and collaborating with colleagues and staff of other health and social service agencies
  • Facilitating communication between community healthcare, community service providers, and caregivers/ participants to best meet their needs
  • Serving as a representative of the program to the greater community
  • Facilitating communications and collaborations between agencies and programs to meet participant needs in program service delivery
  • Recruiting and evaluating candidates for participation in the programs and services provided
  • Recommending and implementing intervention
  • Controlling the general environment
  • Maintaining and evaluating vital logs
  • Abiding by and upholding Personnel Rules and Regulations and Policy and Procedures, including facility, Division and Department policy and procedures
  • Maintaining confidentiality
  • Listening carefully and providing courteous answers to questions from participants, staff, and public as will be determined by participant, staff, and public feedback either verbally or in writing
  • Performing other duties as assigned
