Administer evaluation tools, make recommendations and Identify health related needs of candidate seeking adult day care services
Counsel and train caregivers, participants, volunteers and interns on specialized health care prevention and self -management promotion
Promote preventative health education
Supervise and assist participants with medication administration and health treatment needs.
Observe and evaluates participant level of functioning and provide recommendations for level of care needed.
Monitor and enforce disease control services within the facility.
Implement and manage standard precautions and infection control guidelines.
Train and demonstrate standards of precaution to staff, interns, and volunteers.
Performs inspection, screenings, teachings and counseling in this area.
Report all new suspected or confirmed cases of infection to the County Department of Health and Wellness, Director of Epidemiology.
Participate in investigations of disease control efforts.
Review participant medical records and information to reduce the contraction of airborne diseases in the ADP area.
Generate documented service and maintain records for the program participants
Observe, monitor, and participate in the evaluation and maintenance of stabilized health and functioning of seniors in the program.
Promote, teach and improve individuals desire and abilities to perform self-care or maintain care through the use assistance of available staff.
Evaluate and document current status of participant care.
Generate, promote, and execute on participant and facility safety standards.
Coordinate collaborations with sister adult day care programs to provide additional services if necessary.
Facilitate coordination of individual care plan with service providers.
Assist the program coordinator in providing guidance and direction of adult daycare staff, contract staff and volunteers / interns
Clarify goals, objectives, policies and procedures to all assigned personnel assisting in the Adult Day Care program area.
Advise and train new health care staff and aides to individualized participant programming and service delivery system.
Demonstrate, evaluate, train and coach staff on performance of healthcare services.
Review and clarify and document health related documentation on all participants monthly.
Review, evaluate, improve and facilitate the overall quality of service delivery to participants and caregivers.
Prepare environment for daily activities (i.E. Change calendar, thought of the month, open doors etc.).
Coordinate and collaborate with colleagues and staff of other health and social service agencies
Facilitate communication between community healthcare and community service providers and caregivers/ participants to best meet their needs.
Serve as a resource/ information representative/ expert of the adult day program to the greater community.
Facilitate communications and collaborations between agencies and programs to meet participant needs in program service delivery.
Recruit and evaluate candidates for participation in the programs and services provided.
Recommend and implement intervention.
Control general environment.
Maintain and evaluate vital log.
Abide by and uphold the County Personnel Rules and Regulations and Policy and Procedures, including facility, Division and Department policy and procedures to include maintaining confidentiality.
Listens carefully and provides courteous answers to questions from participants, staff and public as will be determined by participant, staff and public feedback either verbally or in writing.
